Policies/Procedures:

Procedures for scheduling appointments and for registration vary by type of service, and change from time to time. It is recommended that patients call to inquire about current registration procedures.

The registration process may include eligibility verification. It is recommended that patients should bring documents with them to establish identification, insurance status, and proof of residency. If patients are uninsured, they should also bring documentation showing proof of income. All uninsured patients will be screened for coverage and insurance programs that help cover the cost of care.

When patients are registered, they are given a blue identification card. The card will be imprinted with important identification information and should be saved for future visits. There is a charge for the replacement of lost identification cards.

Cost/Fees:

All uninsured patients will be screened for coverage and insurance programs that help cover the cost of care. Fees vary based upon the type of service(s) being provided and are generally adjusted according to income and family size. Services are available, regardless of ability to pay.

Appointment Availability:

Most medical and dental care is provided on an appointment basis. Call for information regarding drop-in services hours and procedures. Medical and dental patients are encouraged to schedule appointments in advance. In most cases this will result in faster services and less waiting time.
