Policies/Procedures:

New patients should contact our Patient Services Department at (510) 981-4100. All uninsured patients must pre-register in person at Patient Services and/or certain clinic locations prior to receiving a medical appointment.

Our pre-registration process will help us assess which sliding fee program you may qualify for. At this appointment, you will be required to bring the following documents:
1) picture identification,
2) proof of Alameda County residency,
3) proof of income within last 30 days, and
4) proof of citizenship (if appropriate)

If you need clarification or better understanding of types of documents that will be accepted, please call ahead.

Alliance Complete Care, HealthPAC, Medi-Cal, Medicare, Private Insurance

Cost/Fees:

LMC offers sliding fee scale programs for patients without health insurance based upon program availability and eligibility such as family size and income.

Appointment Availability:

We offer routine appointments and same day appointments. Patient must call for an appointment.

We serve older adults over the age of 60 years old. If a senior is homeless or disabled, we will see the patient beginning at age 55.
