Policies/Procedures:

All should contact the ACMC Ambulatory Call Center at (510) 437-8500 to become a new patient or to schedule an appointment. All patients will be screened for health insurance/coverage programs. Patients should bring current documentation including identification, proof of income, and proof of residence to all appointments.

Alliance Complete Care, Every Woman Counts, FamilyPACT, HealthPAC, Healthy Families, Medi-Cal, Medicare, Private Insurance

Cost/Fees:

ACMC offers financial assistance to patients that qualify based on level of income and residency requirements. Most patients whose level of income is less than 350% of the Federal Poverty Level guidelines will receive a discount from full charges. Patients whose level of income is less than 200% of the Federal Poverty Level guidelines may be eligible for HealthPAC (Health Plan of Alameda County) which covers the full cost of care with only small co-payments. ACMC’s mission is to maintain and improve the health of all County residents regardless of their ability to pay.
